St. John’s Episcopal Hospital has launched Innovative Health Partners (IHP), the first Rockaways and Five Towns-based independent provider association. The hospital, which has outpatient practices throughout the Rockaways and in the Five Towns, is seeking to build relationships with independent providers to improve access to care, enhance care coordination, and streamline referrals.

“Our objective is to ensure our patients and community members achieve optimal health, and IHP will provide an enhanced platform to support our efforts,” said Chief Executive Officer Jerry Walsh. “Whether patients receive care through an independent provider, or at SJEH, they will have access to the entire IHP provider network.”

Benefits of IHP to independent providers include electronic integration of all IHP clinical practices, streamlined referrals, expedited post-care follow-up reports from SJEH, continuing medical education, access to St. John’s medical library, and more. IHP will encourage the sharing of best practices, problem-solving, and experience, allowing providers to pool resources to manage patients efficiently and effectively.

“The goal is to always provide the right health care in the right setting at the right time,” said Assistant Vice President of Managed Care Donna Skura. “When each person receives optimized care, patient by patient we build healthier communities.”
